Canvas New Quizzes Issue: Student Image Uploads Not Displaying

April 18, 2025 8:15am update:  This issue has been resolved. Please see the recent incident report from Instructure for more details.

On April 17, 2025 we became aware of an issue with all Canvas New Quiz activities that prevents student submitted images from appearing in their attempts when viewed through the SpeedGrader or Moderate quiz screen.

When students are taking a New Quiz assessment on Canvas that requires them to upload and embed an image into an essay question type, the image uploads successfully on their attempt and appears correctly for students. However, when an Instructor or other course grader goes to the SpeedGrader to grade the quiz attempt the image does not appear. If checking the Moderate page for a student’s attempt, the image also does not appear on their attempt as well. However, the image is visible on the attempt if you check the Quiz Logs off of the Moderation page for the attempt so the image is preserved in this way.

The Poorvu Center’s Educational Technology team has alerted Instructure, the providers of Canvas, about this service function disruption. We will update this posting with new information as we receive it from Instructure about the status of this issue.
