The Canvas @ Yale request forms have changed in our effort to transition from Qualtrics based request forms to the new Form Assembly tool. All forms are still available, though the direct URL to the original requests has changed.
We have established redirects for all existing forms to the new Canvas Request forms. Please update any bookmarks or saved URLs for Canvas request forms that you may use frequently.
